View Incident displays Reported By as "org.couchdb.user:username" instead of actual username

🐛 Bug Report

When viewing an incident the Reported By is shown as org.couchdb.user:username instead of the actual username

To Reproduce

Steps to reproduce the behavior:

1. Click on Report Incident menu item
2. Enter Incident details and save
3. View incident and notice that the Reported By displays org.couchdb.user:username instead of your username

Expected behavior

The "Reported By" should display the name of the user who reported the incident

• node version: 10
• fastify version: >=1.0.0
• os: Windows 10
