(feature request): Add in the `created_at` timestamp to the `cash` table #1137

The cash table currently allows users to specify any date they want in the cash form, but we have no way of tracking when the payment was actually recorded in the system. For auditing, it would be incredibly useful to compare dates and see if users were going back to correcting records made previously.

This commit adds the created_at timestamp to the cash table. Closes IMA-WorldHealth#1137.
