As a physician caring for this patient I want to see an up-to-date summary of the patient's problems and course so that I can quickly understand the patient's situation and treat them with an understanding of their whole context
Example scenarios:
The daytime hospitalist (general hospital physician) is handing off his list of 18 patients to the overnight hospitalist, and he has only 10 minutes to make the hand-off.
The intensivist (intensive care unit physician) has 4 patients to admit to the hospital, and they all have urgent need of orders. She needs to quickly understand what is going on with each patient, and she needs to know if one patient has a higher-priority problem than another patient.
The hospitalist is discharging the patient to a nursing home, and she needs to write a discharge summary with all of the patient's current problems.
The gastroenterologist has been consulted for colonoscopy, and she wants to quickly scan over the patient's major problems to see if there's anything that could affect the way the procedure is performed
Acceptance criteria:
Combine problems on the list that are referring to the same problem
prioritize the list according to the provider who is viewing it (e.g. GI problems higher for GI physician)
show relationships among the patient's problems to highlight interdependencies
prioritize problems related to highest risk of the worst outcomes like death
