After the initial extension download, try to do debugging before a reload window and it will fail without giving a reload message. Also, making a launch.json active without first opening a C++ file fails to show the reload message (in regards to the debug configuration snippets requiring a reload). Not sure why we didn't get any complaints...I guess people just reloaded.

Fixed with 0.14.5.
