A description of the issue

I am having issues with the polymorph potion. I tried to drink it, but when I did, I got kicked from the server. I get this error message:

I can no longer log on to the server, so help would be appreciated. Whenever I log on, it throws the same error message. I also submitted an issue with the impersonate mod as I wasn't sure if this was a bug from there or from here.
