automatically change Streams host's destination port

I have a VPN container which features port forwarding. The port changes on every boot up and the port number is saved to a text file. I want to set up a Streams host in NPM who's destination port automatically changes to the VPN's port forwarding number when/if it changes (or at the very least when NPM boots). How do I accomplish this in NPM?
