Let user update license/certification or contact info for existing enrollment

Currently, once an enrollment has been approved, providers, service agents, and service admins cannot change the license and certification information or the contact information for an enrollment. We should let them do that.

It would also be good to let a provider update their license/certification info or contact info for all their enrollments in one go, and for a service agent or service admin to update all a provider's enrollments at once.
