as a follow-up on PRImA-Research-Lab/prima-page-viewer#15 (comment)

If @index is not required to be contiguous (which I would be happier with), please let me know.

- require @Index to be contiguous across types
  via documentation
- improve description of different types of
  reading order groups and their relation
  to each other
...by defining simpler types _before_
derived/nested types
