I've been getting a similar issue to this old one from BungeeCord since one of the recent updates of Waterfall. Easy way to test it is to switch networks via a vpn or other means while connected to a proxy, that seems to break it until you restart your game. All servers this issue occurs on are modded 1.12.2 sponge servers.
