The button next to the left of the 'Server Name' column points to the wrong location. Looks like it is missing /panel/ for me.

Doing: /servers.php?goto=
Should be: /panel/servers.php?goto=

Can you confirm what page this is happening on? Because the link on the server view is as follows, so it should be linking correctly: https://github.com/DaneEveritt/PufferPanel/blob/master/app/views/panel/servers.html#L45

Admin Settings>Find Server>All Servers

The gauge looking button to the left of the name.

I fixed it at some point while working with ORM updates.
