The qt590-r2l branch has work-in-progress support for R2L rendering of menu items rendered fully be Qt (context menus and those belonging to non-native menubars).

The big hurdle is still the placement of the menu item text; checkmarks, icons and mnemonics are drawn at a reasonable location:
