When someone submits a report, a notification should be sent to a slack channel with the following details:

That a report was submitted
The user who submitted it (email address)
The organization they submitted on behalf of
The encampment visited
A link to the URL of the report

The Slack URL and token should be configurable per instance of this app, and the URL may be different from other slack notifications.
