OpenPHT cuts video after a commercial break fade, shuts down

I'm having a weird issue that just recently started. When a TV show video does its commercial fade in/out the video doesn't come back on, and then OpenPHT shuts down. I can still hear the audio, however.

I also have this happen a lot. It seems to be related to overall scene blackness percentage. I've had it happen on commercial fade outs and also during opening credit sequences where there's just a little white text on a black background.
