add extra visual notification of blocked requests

There should be an option to enable extra visual notification of blocked requests, such as flashing the statusbar icon when it changes to indicate blocked content.

I'm not sure I like this one because I really don't want the menu to be busy. Maybe this will be an off-by-default option down the road after higher-priority features are implemented. If people would like this, they will hopefully let me know so that I know this has general appeal.
