The CTags plugin should support user-defined tag file names.

Currently changing the tag_file directive to anything other than .tags, e.g. TAGS, causes the Show All Symbols menu not to be displayed.

Changing the CTags configuration file to use .tags as the filename fixes the problem.

I'm using Sublime Text 3 (3065) and CTags (48f1cd3). The CTags distributed with Package Control is affected by the same problem.
