Feature Request: refactoring descriptions

Could you add quick descriptions to the refactors in the readme?
I don't exactly know what they do (I could look it up in the book but you can't expect every user to be able to do so).
for example for extract function:

You select a code snippet which then gets extracted into a function and replaced with a call to that function.

So just a quick descriptions what they do and how you use them

And, I don't know if this is possible or how hard, it would be rly cool to have these descriptions in the preview window in the refactors telescope pickers.

I think having the descriptions show up in the preview window will take a bit more work, but for now I've just added it into the README.
