I'm getting this as well. HL2DM Gets it even more often.

This happens in most Source games right now. I notice it quite a bit in Portal when mucking around with guns.

Still present in latest build of HL2:DM, though I'm unsure what exactly starts it. I had to join a server and play for a bit before it occurred.
