Tracking issue for islands. Available in the tiles as point features (representing some sort of centroid).
Only available from z8, which is only useful for the smallest of islands. Needs OMT fix. There's also ‘rank’ but an explicit numeric value of “what’s the area in fractions of a map tile” is probably the most useful. osm-carto won’t label both Ireland and Great Britain at any common zoom level, for instance.
I think OMT ignores place=islet.
Islands can be coterminous with administrative boundaries at all levels, populated places, and protected areas. In many cases this would land the labels right on top of each other. Adjusting the label placement with text-variable-anchor and text-radial-offset possibly zoom dependent, might be smart.
There's also a place=archipelago multipolygon, but it's not in the tiles and the best use of these shapes might be to generalize as curving text lines.
I won't claim to have looked at lots of paper maps but italic text for natural features is also used for water features, feels right.

Unlike islands, most of these are mapped as nodes instead of areas, which makes it difficult to gauge their relative importance. Rendering labels relative to these features' size and extent in Americana would incentivize mappers to convert more of them to areas (like I did with the Ungava Peninusla).
