Should all ivy--*-buffer-action call ivy--reset-state? #1741

If I call ivy-switch-buffer and rename a buffer via ivy-dispatching-call, the new name is not reflected in the buffer list. Similar behavior occurs if I visit a virtual buffer, i.e., it is still shown as virtual even though it no longer is.

On the other hand, if I kill a buffer in ivy-switch-buffer, it gets marked as virtual, but afterwards I cannot re-visit it and get a "Selecting deleted buffer" error message.
