Pasien Data persist on ID Pasien change #12

To reproduce this bug, follow this step:

Go to Add Pasien Form
Insert a valid Pemeriksaan ID
Insert an existing Pasien ID
Data of that pasien will show up
Change the Pasien ID to a new Pasien ID
The data from the previous pasien will still persist in the form

Ideally, when Pasien ID is a new one, the form should reset and display a blank form for the admin to fill
