Capture more information on signup.

Capture default location of volunteer/user.

Add a checkbox for specifying whether it is a user/volunteer.

Ask for age of volunteer. We cannot have people under age of 18 or above 60 volunteering.

Make sure that we link to WHO and any other relevant documentation for protection. (May be even their local language)
