favicons generated with this don't work in Snow Leopard & Old Safari

Looks like there are some incompatibilities with the 16x16 favicons generated with this library and older versions of Safari and/or older Mac OS X. Like Safari 5.1.1 running on Snow Leopard 10.6. They just show up as fully transparent images.

Any ideas what might be wrong?

One explanation for this might be that older browsers need the BMP data, not the PNG data. Another possibility is that a 256-color 16x16 layer is required.
