when I download the zip-file and try to open middleclick, I receive from Sonoma 14.3.1:
"MiddleClick" can't be opened because Apple cannot check it for malicious software. This software needs to be updated. Contact the developer for more information.

After moving it to your applications folder. Right-click it and press open, then you can bypass this screen.
Alternatively, install it with Brew

Alternatively, after you get this popup screen, you can close it, visit your Privacy & Security tab in macOS's Settings and there you will see a new section that allows you to allow this app anyway.
This is actually true for all mac apps that give you this popup, not just MiddleClick.
