Deploy JIRA Connector

This is just a general tracking issue to cover deploying the JIRA connector to the GitHub Infrastructure. More details to come.

We could do this using GitHub infrastructure or replicate what we did for Slack by deploying to Heroku. Suggested that for expediency, we deploy to Heroku whilst @andymckay investigates how we move apps onto the GitHub infrastructure.

We've deployed this to a production Heroku instance, but we won't block the initial release on getting this on GitHub infrastructure.

Did we decide to stay on Heroku or are we planning on moving to GitHub infrastructure in the long term?

Let's stick to Heroku, its a known quantity at this point.
