How to remove the footer that is added?

I had a similar problems with GD itself putting a header:
"index.html"
and footer:
"Published by Google Drive–Report Abuse–Updated automatically every 5 minutes"

• how do I remove either the added GD stuff or the footer added by google-docs-publisher?

This feature is not available yet. It will be but in a paid version ($5/year), along with several more features (analytics, customization etc.).

Do you intend to subscribe to this plan? Would you like me to keep you updated as soon as this is available?
