Modified Field Incorrectly Displays Creator Instead of Last Modifier

Describe the bug

In the Subject type, Program, and Encounter type sections, the system is incorrectly displaying the name of the person who created the entry as the last modifier instead of the actual last modifier's name.

Steps to reproduce the behavior:

1. Access the app designer panel of any organization.
2. Navigate to the Subject type, Program, or Encounter type section.
3. Select any entry within the mentioned sections.
4. Observe that the Modified field incorrectly displays the name of the creator, rather than the last modifier.

Expected behavior

The Modified by field should accurately reflect the name of the user who made the most recent modifications to the entry.
