rework exceptions

we should refactor all of our exceptions, and use error instead where it makes sense.

• move exceptions from Exception namespace to Throwable namespace in all components.
• extend Error instead of Exception in InvariantViolationException ( and rename to InvariantViolation ) - extend Error for InvariantViolationException instead of Exception #308
