Passed steps from Scenario Outline/Examples are included as "untested" in the html report. If you change the same steps from Scenario Outline into regular Scenario (I did this to confirm the issue was related to Scenario Outline and Examples), you will no longer see the "untested" category in html report. My conclusion is that behave-html-formatter library has some bug when calculating the passed steps under Scenario Outline.
