The switch wallet code has been affected by recent changes and is not completing the handover correctly. The UX is simply that MBHD disappears and never returns.

This is due to a thread not returning correctly in MultiBit Hardware. I'm progressing it ASAP.

#302 Made the handover future more robust for error handling

Pushed the changes to MultiBit Hardware so this should now be working correctly. It also has more robust error handling overall.

Ready for review and close.
