Bug: Slack Appears as Helper App in App Mixer, undefined Text Displayed in Place of App Icon of Helper, Main Slack App Slider Does Not Control Slack Volume

Describe the bug
A Slack helper app appears in the App Mixer called com.tinyspeck.slackmacgap.helper along with the Slack app itself once audio has been played inside of Slack since its launch. Instead of an app icon for the helper, the text, undefined is displayed and extends into the next column.

Steps to Reproduce
Steps to reproduce the behaviour (feel free to change the placeholder as you need):

1. Open Slack. Just the Slack app will appear in the App Mixer.
2. Play audio within Slack. This adds the helper to the App Mixer. The volume of the Slack media can only be controlled by adjusting the helper's slider and not the Slack slider that has the icon.

Expected behaviour
There should only ever be one Slack slider in the app mixer. When active, its tooltip should read, Slack and it should have the Slack icon.

Setup information:

• Audio device used for playback: Audinst HUD-mx1
• Audio transmission interface: USB
• macOS Version: 10.15.7
• eqMac UI Version 1.17.2
