Please specify:

Total amount of mask batches (min 25 masks per batch)
Model of the mask (It's recommended to print Prusa ones)
Location to be used (need proof of need and later post photos of use)
Ethereum address to receive grant
Specify how they will be cleaned before giving them

Example:

2 batches (25*2)
