Inconsistent naming of dose_indices #7

When a crm_fit is converted to a node on a dose pathway, as in:

https://github.com/brockk/trialr/blob/master/R/dose_finding_paths.R

the slot dose_indices is renamed dose_index.

The name dose_index featured in the JSS submission, so I am stuck with it now. :-(
