when I throw a frag grenade from classic this happens

Most likely your voltz Engine version is outdated. Try updating to the latest alpha of both it and ICBM-Classic. If that doesn't work reply back with the versions you are using so I can check if the files built correctly.

Also, you're on the wrong issue tracker. This is the MFFS tracker but the bug is an ICBM-Classic bug. Its no problem but try to check the repo name next time.
