I hope this is the right place to leave this comment.
I noticed that the documentation in the moodle plugins directory is an exact match to the documentation in the master branch of this repo. But there is a line that is confusing, "This plugin will work natively with Moodle from version 3.8 onwards". Does that actually hold true for the versions in the directory that are tagged for 3.7 and/or 3.8? Or is the master branch documentation pulled directly from github to the plugins directory listing? My assumption is that the docs don't match the plugins directory listing.
If I did install it on Moodle 3.9, I would pull the master branch and place it in the .../admin/tools/security questions as described. It is still concerning/confusing as to if that would work as expected though since there has not been an official release since 2020. But the documentation leaves me mostly confident.

(Admittedly, as I wrote this I realized that I did not check the file permissions and ownership. That was probably the issue. I have to wait to try again.)
I tried following the installation instructions on a Centos 7 host and got stuck on updating the database. Site would not return from request. One thing that I did that might be off is that I first pulled the repo to the home directory ~ and then did cp -r ~/moodle-tool_securityquestions path/to/moodle/admin/tool/securityquestions. Should I have moved the whole folder to under the securityquestions folder?
Also it looked like in vershion.php it was necessary to match the release to the version:
$plugin->version = 2023020700;
$plugin->release = 2023020700; // Match release exactly to version.
Not sure what effect that actually has on install if any, but the comment seemed clear.
