Move the "didn't find your sidewalk" form into the AAS app

Should be presented within the Adopt-a-sidewalk app, as a modal. Perhaps the city can provide a stripped down, iframe-able version for us to use?

Also, updates to the field format:

a. Remove the colons
b. Use sentence case
c. Get rid of 1. 2. 3, including the sentence.
d. Visually differentiate between ‘send’ and ‘clear’ buttons.
e. Re-label ‘Send’ ‘Add your sidewalk’
