Call hierarchy items for member functions should include class name #1185

In the following code:

void bar();

struct Waldo {
  void find() {
    bar();
  }
};

struct Locator {
  void find() {
    bar();
  }
};

if call hierarchy is invoked on bar(), the names of the call hierarchy items for the two callers are both just find. It would be better to include the class name as well.
