ntp plugin seems to have plugin_instance/type_instance backwards

Looking at the peer info from the ntp plugin, it seems that each peer should be the plugin instance instead of being in as the type instance. This would be an incompatible change but if people agree, I'd do it by adding an option to let people fix so that we have an easy path for the next major release
