Describe the bug
The Notes breadcrumb button on Patient Notes page links to an empty white screen

To Reproduce
Steps to reproduce the behavior:

  1. Go to https://care.coronasafe.in/facility/caaab916-e877-4153-a0a4-d0afc69f4c1a/patient/d8fd245a-c435-4bca-83fc-4c39cc6e9aae/notes/
  2. Click on Notes

Expected behavior
The Notes button links to the same page, so nothing should change
