Hide the Query Parameters in the patient tab

Describe the bug

when we are opening the patient tab for the first time, in the url it is showing the query parameters as " https://care.coronasafe.in/patients?page=1&limit=12&emergency_phone_number= "

Steps to reproduce the behavior:

Go to : https://care.coronasafe.in/patients?page=1&limit=12&emergency_phone_number=

Expected behavior

when we are opening the patient tab for first time , it should only showing " https://care.coronasafe.in/patients "
