Unable to create a new user in care

Describe the bug

When we are trying to create a new staff user, using a district admin id, the new user creation form is not getting submitted

To Reproduce
Steps to reproduce the behavior:

1. Go to https://care.coronasafe.in/users/add
2. fill the column and click add user button
3. See error

Expected behavior

when we click on the add user button in the user creation page, we should be able to create a new user
