Notes filled on discharge patient pop-up are not reflected in patient consultation page

Describe the bug

When we discharge a patient from care with a note, only the discharge reason is shown and the discharge note is not been shown on the patient consultation page

Steps to reproduce the behavior:

1. Go to https://care.coronasafe.in/facility/53a7266a-ed34-4616-85eb-7ae1be57d2cc/patient/4ae3b7de-800d-4b41-8ac9-00dbb741fe9f/consultation/54959916-9496-49ea-9ebe-e1ff579e5557

Expected behavior

Create a card in as the first component on top in Info with the title "Discharge Information"
The entire content of the discharge pop-up window, based on reason of discharge must appear in the box.

1. if patient is discharged with discharge reason as recovered, the following content must come:

• Discharge date
• Discharge prescription
• Discharge advice

2. In case of Expired:

• Cause of death
• Date and time
• Confirmed by

3. Referred:

• Notes
• Date of discharge

4. LAMA:

• Notes
• Date of discharge
