Describe the bug

the input pop-up for the pain scale inside the vital page is misaligned

To Reproduce
Steps to reproduce the behavior:

  1. Go to https://care.coronasafe.in/facility/c75558b6-d357-4bbc-8463-e1b8c649e267/patient/5a34a9f9-7d43-4343-b8f2-fc82b70e4128/consultation/3d9f49b9-ef35-4c3c-8591-d7924af7d8dd/daily_rounds/15edb8c5-9ec0-4103-85ff-17ae6c661428/update
  2. Click on vital
  3. Scroll down to pains
  4. See error

Expected behavior

It should be properly aligned
