Describe the bug

"something went wrong" error hits in, when we are trying to create a new shifting request

To Reproduce
Steps to reproduce the behavior:

  1. Go to https://care.coronasafe.in/facility/ff83ce45-3229-4c1d-96e0-2b4b0ee5992f/patient/3f2b9444-f04c-49ad-863c-feb7a4699ebf/shift/new
  2. Click on submit after filling all datas
  3. See error

Expected behavior

We should be able to create a new shifting request
