Bug: Unable to erase "referred to" hospital

Describe the bug
When i discharge a patient, in the discharge pop-up, when reason for discharge is marked as "referred", and I select a hospital in the "referred to" field, I am unable to then clear the field

To Reproduce
Steps to reproduce the behavior:

1. Go to Discharge pop-up (https://care.coronasafe.in/facility/a3910fb7-db3d-4ddf-aa39-1f20e61d1453/patient/7af2ae4b-97e5-40e5-a585-df820de2c8dc/consultation/0ce602fd-5ace-4e84-933c-522087b8fb8f)
2. Click on 'reason for discharge' select "referred"
3. Add name of a hospital in the "referred to" field
4. try clearing the "referred to" field. User is unable to do so

Expected behavior
Allow user to clear the "referred to" field, since it is anyway a non-mandatory field
