Patient Transfer is malfunctioning in patient creation form

Describe the bug

When we try to transfer a patient using the patient registration form, the patient is getting transferred to the same facility irrespective of the facility selected in the initial pop-up

Steps to reproduce the behavior:

1. Go to '...'
2. Click on '....'
3. Scroll down to '....'
4. See error
