Patient transfer is malfunctioning

Describe the bug

When i use the patient transfer in the patient registration form and transfer a already discharged patient, to a facility, the patient is getting readmitted to the old facility from where the patient was discharged.

Note: If the patient is live in any facility, it is working properly.

To Reproduce
Steps to reproduce the behavior:

1. Go to care registration form
2. transfer a discharged patient to a facility
3. See error

Expected behavior

The patient should be transferred to specific facility based on the selection in the facility select pop-up
