Patients not admitted- remove Bed status

Since beds are not assigned for patients in "domiciliary care/refer to another hospital/OP Consultation/Declare death", remove the option for all patients other than the ones marked as "Admission" in Decision after consultation in consultation form.

• Remove 'bed status' section from edit consultation form
