Add support for Travis CI. Update the project's README to show the latest build status.

To make the Travis CI test coverage extensive, the integration tests should be converted into unit tests. See #21. Otherwise the Travis CI build has no way to test any of the command logic because the Travis CI build agent cannot run the CouchDB server.
