Followup to #118.

Now that the structure of LoginManager is better we should clean up the user interface:

Get rid of spaghetti and replace by something new
Get rid of icon and replace by something new
When used through HMD with pointcloud representation, "invite" users to self-calibrate
Show that we're in auto-login, and where we are
Rationalize settings
