The Sky Q box is running on a different port number (5900).

I don't have a Sky Q box to test on at the moment - does this work OK with just the port number changed?

Yes it does. It also works on the Sky Q Mini.
The buttons on the remote are different, but they re-use existing codes.
These are:

New Button Old Button
HOME TV_GUIDE
DISMISS BACK_UP
SIDEBAR INTERACTIVE
SEARCH SERVICES
